Hi Geeks

I need some client consultation forms, I do waxing both general and intimate, eyelash extensions, spray tanning and eyelash and brow shaping and tinting.
Would anyone happen to know where I can get them from? I've had a look in Capitals and online but I've not really found anything?

Why not do your own? That way you can adapt them for all the treatments you offer on one sheet. HTH x
 
I think ellisons has ones for each of your treatments but as hoggers says you could make up your own multiple treatment record card type forms.

Thank you girls. I did think about making my own ones but am wondering of there is anything mandatory that should be on them, I don't want to put to much/too little on them xx
 
If your with babtac they have their own consultation sheets that they suggest you use. X
