Customer consultation forms-help

hi all
looking for some help please,i have recently done diplpomas in threading and lash and brow tinting am looking to go mobile. i have been looking on the net for customer conusltation form templates and also a disclaimer form but not having any luck!just wonderd if you ladies could point me in the right direction.thanks in advance zarz x

We're you not given them as part of the training course, I would go back and ask the training centre for a copy of theirs

I agree with that. You goon a course,you expect to come away with the info you need, and that includes templates or info to make your own.

i would add tho becarfull with standard templates. Use themby all means [why re-invent the wheel] but check the wording. Your insurance company will probably have there own ideas on what you should advise clients and what information to collect. This varies from insurance company to insurance company, so make sure your template covers everything YOU need.

Also make sure you don't collect info you don't need - its naughty lol
