Here's my forms. I am not claiming any copyright (I think they are derived from my course notes Andy - thank you!). Ladies; it's written with blokes in mind - so the question "Are you pregnant?" has been removed!

Using 200gsm paper, I print the Customer Record on the back of the Consultation. The nice bit is that my price list is on the bottom so I can easily tot-up the bill as I record the treatments.

I also list patch tests and any relevant notes here. Returning customers just get asked if there is anything changed on the consultation.
