I am a wee bit confused here, which isn't hard.
Do you mean if a client indicates they have a problem on the consult and you go ahead anyway? On my consultations for all treatments I ask all the questions and mark Y/N and the client signs it. I don't consider this a waiver, if a client has answered yes to a condition and is contra indicated I refuse treatment and mark it accordingly on the card, and offer if possible another treatment that they are not contra indicated for. This is exactly what Andy also reinforced at our workshop on Monday. The way I have read this bit is that a Consultation is a Waiver, which is not what I use it for. |

EDIT: A few messages have been deleted since this was first posted, but a good point had been raised about waivers and the advice still stands.
Put simply, your consultation form cannot be used to waive the rights of your client. What it can do is help you make an informed decision about the proposed treatment, i.e. whether it is safe and appropriate to wax, whether you need to adapt your treatment plan, or whether you need further information or approval from the client’s healthcare practitioner before proceeding. The declaration at the bottom of my consultation form serves 4 purposes:

Hi I have designed my own client cards as there would be too many for all treatments. I include not a waiver but a paragraph on informed consent. This means that if the client is not contra-indicated and proceed they know that some treatments have temporary side effects. This I believe is what Andy is trying to say. Further info on particular treatments discussed is also added to the treatment log.
informed consent: I understand that after some treatment procedures the following observations may occur: Minimal skin discomfort for a short duration of time - Dry patches may appear with slight flaking of skin - Temporary skin reddening may occur - Possible breakouts which can be managed with home care It is recommended to follow home care advice and to use the appropriate home care products. |
