Ok so at our salon we have had a couple chemical services not turn out do to people using at home chemicals, color and ect, I was wondering if any of you would post your release forms that you have clients sign before a professional chemical service. Im needing some ideas so we can make one up.

Release forms go not cover you!
 
Surely if there is any doubt as to whether the service will work a strand test is carried out first?
 
Take strand ltest if there's a chemical reaction to something you shouldn't be afraid to say no or inform your employer (if your not the salon owner) that you dot feel it would be professional to preform the service on hair that has had a reaction. It also depends on what brand of colour you use.
As people have said release forms are not worth the paper they are written on :)
