Following on from the patch test post....

I patch test every single colour client of mine including my partner, best friends etc etc....so what I do is have them sign my client record card which is simply a 6x4 index card with their name, address, telephone number and date of service on. Is this enough?

Is there a specific form they should complete? I am old school and believe some of the old (simpler) ways are the best but am I covered enough?
