I just buy the client record cards from Ellisons (other wholesalers do them too) - these cover the essential info for most beauty treatments. Plenty of space to write down anything extra you might want to record about the client and the back of the card is lined to fill in the details of each treatment you do.
If you are keeping everything on computer then i'm sure you could create a document or detailed contact listing.
Perhaps someone on here can advise about forms you can fill in using an ipad in front of the client?
