Blank COSHH sheets

Hello,

I've got together all the MSDS sheets for my products and I, now, need to write COSHH sheets for the hazardous ones.

I've searched the web and found several examples of blank COSHH sheets, but the details required on them seem to vary wildly! Some are so simple that they can't be legal and others are so complicated it looks like everything from MSDS has to be copied onto them!

Can anybody show me, or direct me to, an example of a blank COSHH sheet that would satisfy a Health and Safety inspection, without being unecessarily complicated.

i'm pretty sure that if you fone the company up they will send you the coshh sheets for their products. can't remember where i read this but just give them a ring
