Client consultation forms

Hello, I was hoping that someone could help me please?

I have recently qualified as a nail technician and want to make my own client consultation forms but I'm really struggling to do it. Does anyone have any tips or advice please?

Your insurance company may have a template of forms you can use on file. On with BABTAC/Balens whose forms I use.

Did your training provider not supply a form? Mine did as we had to complete them as part of our course and exam
