Client Record Cards & Aftercare Leaflets

Hi all
Does anyone use anything like a guide sheet which are given out to the clients prior a treatment (or even after if it has aftercare advise)?
In anticipation of my first clients (one certs and insurance is received), Ive been playing around with a few ideas which I thought might give that little bit extra customer service.

Any thoughts?

Hi all

I know the basics of a customer information sheet but I cant seem to find anything like what Im looking for.

Now I might be trying to make this more complicated than it needs to be but basically Im looking for a sheet which has all the normal info on it along with comments section but also 'hand prints' at the bottom so that I can mark any issues at the time on the right finger as well as making notes.

Has anyone seen anything like this?

i've not seen a handprints one hun but the ones i get from capital are quite good,they have a chart section kinda thing with a space for each nail to say condition i.e n = normal bi= bitten etc....

i certainly give out aftercare advice leaflet and know alot of other techs on here do the same,if you do a search i know there are at least a couple of threads with suggestions as to what to include in there,mine is on my works computer not this one so cant upload im afraid.
