Colour help please

Asking on behalf of my colleague as it's got us both stumped!

The client just wanted her roots highlighted to match the mid and ends, it hadn't been coloured for a year.

My colleague chose to do bleach with 6% and igora 12-1 with 12% alternate foils.
The foils were left on for a bit longer than maximum development time but the hair didn't lift very well and you could see a clear line between the colour and the midlengths.
The hair was in good condition still so my colleague did a second application of the bleach foils with a higher peroxide but it just wouldn't lift anymore.
In the end she toned it with a 10-1 which obviously didn't do anything as the hair didn't lift to a 10.
My colleague asked me what I would have done differently..
The only thing I wouldn't have used is the high lift just because I find they throw a lot of warmth out anyway but I have no idea why the bleach didn't lift the way it should :-S
Does anyone have any ideas? The client wasn't pregnant or on medication so I doubt hormones would be the cause. Hope pics work!

Highlift was the problem, then when shes rehighlighted i don't think shes put enough in.
 
I totally agree she had to much of a regrowth all pre lightner and more of them
 
I could understand the high lift not lifting well but thought the bleach would have lifted lighter especially after doing it the second time.

Its clear she hasn't put enough in, you can see theres like one highlight thats light enough!
 
Should have just used bleach with 6% not a highlift the hairs too dark
 
Well if it was myself judging by the pic she looks a 5 so when lifting its going to produce yellow tones so straight away I'd use alfaparf super mech + with 20 and finish with 30vol to lift evenly from this after pic it the highlight was a wrong choice as it threw back to much warmth , and she did not blend the packets down enough onto the old highlights causing a shadow ..as for doing it s second time does she space out her foils a lot and did she use highlight a second time?? Will she be doing it again?
