Combining beauty consultation forms

Hi I’m really struggling on what’s the best way to do consultation forms. I offer multiple treatments but doing a form for every single one is a bit over whelming including the patch test form & client card. What does everyone do if they have lots of treatments they carry out? Is there a way to combine them into one form without it being an essay? Thankyou
 
I created my own form because I had the same problem. It's A4 that gets folded and still fits in my consultation box. Works well for me. The thing is that we do need almost an essay from our clients to protect ourselves.

On the front of the form is every mortal detail that is needed from our clients regarding all the usual things. There is a covid declaration and gdpr addition consent options.

Back of the form is a list across the top with all the services that I offer and they are circled as to which treatments they receive.
Lower on the form is the date , service on the day provided, anything remarkable or products used, cost, initials of therapist. job done There is plenty of space for multiple dates and appointments, so they work well if the client comes frequently.

The only other difference is that I do have a separate lash lift card, proving test history etc.

You could maybe do one form that is more general beauty based, and another that is more holistic based if those are your preferences.

I have so few clients left now after the terrible year we have had that I have no problem with storage of the forms. It suits my business as I am a sole trader.

Thankyou so much Rosie! I feel one big form is the best to go. I specialise in lash extensions mostly but would be so much easier just to have it all on one form rather then lots of paper!
