Consultation forms minefield

Hi all,
I am looking for advice regarding legal paperwork.

when I have a new client who is receiving anything that involves tint I always carry out a test patch. I have consultation forms, I also have client record cards. Is there not a simple way to combine everything. I really feel overwhelmed with all the forms available.
do I need a new form for each different treatment :-/

below is what I currently have but also have a consultation form for facials, peels, tint waxing etc it seems like a lot to get them to fill in each time?

Hi if you download Timely Consult it is free and has everything that you need. We started using Consult a few months ago and it’s super easy. It is in our salon IPad and we sign on behalf of the client .

I was using faces but I’m so confused between disclaimer consultation and client record
