consultation forms

Hi,

Anyone know where i can get decent consultation forms for hairdressing services ( from cutting , colouring and extensions ) from. I know i could make my own but my word processing skills is terrible and i want them to be professional.
I would love to just download and print !!!
Any ideas folks

Raccoon do pro consultation forms which are in duplicate, so the client has a copy, and the salon retains one. They are really good. Worth a google.

Lots of the hair companies supply salons with consultation and skin test record material now. We are redken, they give us tons!

I think habia do client packs, i would presume these have consultation forms in. You do have to pay for these though
