Contract layout for technician/salon

Hi, quick question. Im looking to work as a percentage base income between myself and the Salon, the salon has never created a contract between the technician and themselves before.

Could someone please advise me on how this is to be laid out so I can let her know.

Im doing 3 days a week, 25% to the salon. What would in this contract?
