Declaration - Making your own for consultation cards

Hello,

I have standard consultation cards from capital for all of my clients, and anyone that has a treeatment / consultation card completes this in full...

But..

I was thinking about a lot of other things that should be included, and wanted to make a seperate sheet for added info...

So I have asked things like............

Do you suffer from diabetes? Yes/No

Do you suffer from epilepsy? Yes/No

Are you pregnant or suspect
You might be pregnant? Yes/No

Do you suffer from any medical condition or are you under going any kind of medical treatment Yes/ No
If Yes, please specify and name any medication you are taking including HRT

I wanted everyone to look at my declaration also, as I don't know if I'll be covering myself, and as we all know with things like this, we all need to be doing that.

What does everyone else do with their cards... Taylor make or use standard ones?

I put a thread on here a while ago regarding medical dictionairies and have started to use one for looking up and referring to bits.

Can you all give your feedback on this before I go & print a load off.

The information I have provided on my client consultation cards if true to the best of my knowledge. I will inform you of any changes to any of the information required from me, prior to any future treatments.

I give my full consent for any medical conditions /medication to be researched as and when necessary, and understand this is solely for the purpose of client care and for any contra indications that may affect my treatments.

The information on this card is confidential and will only be used for the purposes for which I was collected and will not be passed to any third parties.

Be good to know what you all do and your feedback.

Has NO ONE got a declaration on their client cards then?!

I wanted to know how to word letting clients know I will look up their medication so I know contra indications and complications, and they can sign their card knowing this.

What does everyone else put? Do you use standard cards, or put your own wording?

Please ...? Is anyone willing to share?

Yeah I make my own up. Obvisously theres so many things to add these days especially with regards to allergies. So things such as are you allergic to bananas, nuts, latex etc should be on there.

I do add the declaration that failure to inform (my company) of any changes can risk your health.

However I have an extra sheet attached that asks each time my client has a treatment to declare if the have had any changes and to mention it or sign if there are no changes.

A lot of clients don't realise how significant things can be no matter how small such as being pregnant. So I put if you think you are or may be pregnant please inform the therapist.

I have 2 cards the first is a standard record card, which has their details name address DOB numbers and then dates of treatments and what they have inc retail, the other is for medical conditions and what we agree to do in their treatment they sign this one to say it is all correct.

Are you using that type font for your declaration? Personally I would use one that is really easy to read, you need this kind of thing to be really clear xx
 
I no, I just used that font for seperation from my question!

I think that is the best way to do it, by having your normal / standard one, then taylor making another one.

Do you all use medical dictionaries to look up what they are taking and what side effects etc they might have?

This is partly why I am doing this. I suppose there is no wrong or right things to put / ask. They should feel more confident that you are being more thorough and finding info regarding their treatment, and MOST people don't mind filling out a form for a treatment.

ANyone else got any ideas or tips?

Yes I use the med dictionary, however very few of my clients actually can pronounce or spell what it is they take lol!!!

It depends on their problems i.e reoccuring skin conditions etc when I really probe what meds they are on.

Where does everyone get their declaration's from then? The basic one's you buy have hardly any info on them.... They don't ask about pregnancies, diabetes, epilepsy etc.

Does anyone else have any suggestions to add to mine?

Babe can i be really mean and spot a spelling mistake, top line.Should it be is true. Sorry hun had to tell you before you printed them all off.

Thanks for pointing that out!!

I had a geek how sent me on of hers so I can see what kind of thing people are putting, and you can amend as you feel you need to.

Hopefully, if anything, people are more aware of how basic some cards are and that we all need to ask more questions, especially relating to specific treatments.
