Hello,
I have standard consultation cards from capital for all of my clients, and anyone that has a treeatment / consultation card completes this in full...
But..
I was thinking about a lot of other things that should be included, and wanted to make a seperate sheet for added info...
So I have asked things like............
Do you suffer from diabetes? Yes/No
Do you suffer from epilepsy? Yes/No
Are you pregnant or suspect
You might be pregnant? Yes/No
Do you suffer from any medical condition or are you under going any kind of medical treatment Yes/ No
If Yes, please specify and name any medication you are taking including HRT
I wanted everyone to look at my declaration also, as I don't know if I'll be covering myself, and as we all know with things like this, we all need to be doing that.
What does everyone else do with their cards... Taylor make or use standard ones?
I put a thread on here a while ago regarding medical dictionairies and have started to use one for looking up and referring to bits.
Can you all give your feedback on this before I go & print a load off.
