We all know that pigmentation is one of the leading skin care concerns in our industry. Since we have discovered that dermal pigmentation is a reality, how would we as skin care therapists/somatologists be able to treat this type of pigmentation? Do any of you use a skin care range with which you have really seen a difference in your clients skins? Would microderm actually have an effect on dermal pigmentation?

...keeping in mind that the culprits are normally the macrphages in your dermis in which these pigment is captured...what is the normal life span of a macrophage? And since they are mobile phagocytes, will the pigment then also 'travel' ???

How do we successfully treat this tpe of pigmentation??

It is not a case of one treatment suits all. Hyperpigmentation is notoriously difficult to treat. Some respond well to laser, some to peels. I would say a combination approach is best. Whatever tx is decided upon, needs to be combined with prescription topicals. OTC skincare just will not cut it. Microdermabrasion, in my experience does not really treat pigmentation. The clinical studies are very ambiguous. Can even make it worse - as can all treatments. (There is always that risk).

Recently I have treated patients using micro-needling (medical device not superficial ones). A course is often needed and results have been encouraging - however no instants! We are talking about six months treatment phase.
 
I am actually very carefull...even scared to work on ethnic skins when treatening them for pigmentation. Especially back in South-Africa we have a large number of ethnic as well as asian skin types to treat and with this harsh UV exposure it hightens the possibilty of aggrevating the pigmentation!! Aaarggh....any advice??
