Does anyone have a permission sheet that the clients mother signs to agree to piercing their sons/daughters ears? What way do you have it wrote?

It's called an indemnity agreement. You should be able to find a free template online, then tailor it to suit you.

If you're still struggling, you can post on here or PM me to check. Just to be on the safe side, you can always check with your insurers.

My forms are provided by Studex, who I trained with and whose kit I use.

Which brand do you use and do they not have their own consent forms?

I would check with your insurers sweetie, they may have their own wording which you would need to include, just to be on the safe side!
