Eyelash Tint/Perm Test Consent Form

Sorry if title doesn't make much sense !

Im in process of designing a form for clients to sign and date to confirm they have a had a skin test prior to the treatment but a bit stuck on how to word it.

I thought it would be a good idea to do one as I think its clearer for the client and myself rather than a signature on the back of a record card, so if anything ever happened and insurance involved I would have a signed copy of the form stating the client has had a tint test, understood why they've had it, etc.

Has anyone already got something similar to this ? If so could you share a few things that you have included ? Thanks :hug:

I explain reactions etc verbally to them.

On consult card I have an area

Date

I do similar, except I also have a space for me to sign as well.
