In Australia, Swarovski are the best stones (actually they are the best in the world no matter where you are), they do not sell direct and you have to go through a distributor.

They will design a logo for you, it will be in crystal stones, not glass or plastic and the shine speaks for itself. But a logo by swarovski will cost you

You can get Swarovski hot fix gems I brought mine from the dreaded ebay. I didnt use them on my uniform though I used them to jazz up my old mobile they are expensive for swarovski gems though. I also used them to jazz up my old guess watch strap and a pair of jeans and they do wash up good and go through a tumble dryer fine too.

Check with Anna/Papi from 'rhinestones and crafts' on ebay.
She has TONNES of hotfix stuff... and I 'think" she does logos, though can't be sure.

I jazzed my tunic up with iron on gems of ebay..you can even ask for your name or business name to be made up.
