For Spray tanning..

Name, Tel, Address, Skin tone, Currently taking meds? Allergies? PRegnant?
I understand that i am responsible to inform thr salon/therapist of any medication i am taking or any allergies and skin disorders i suffer from before the treatment beings.

Hard to explain how i've done it by just typig but that doesn't matter aslong as the info is on there correctly
Then same again meds, allergies and pregnancy..along with notes, comments and signature.


I feel i'm missing something out on manicure & pedicure?????
I used one's from beautyexpress before and wanted to make my own now i have a printer again.
 
For me and spray tanning i also ask about any recent tatoos, piercings,skin disorders,cuts/grazes and as well as pregnancy any hormone treatment/menopause. HTH
 
I don`t think a lot of therapists really realise the great importance of carrying out a thorough consultation and actually completing the forms instead of just verbal questioning! I would rather include too much on the forms than not enough. Personally, I have found that some of these consulation cards that you purchase through the wholesalers don`t include sufficient information. I have made my own A4 consulation card with everything on the clients medical condition (past & present), medication, skin types, allergies (again past and present) and I have also included -
" I will inform my therapist if any of my medical details/medication changes from appointment to appointment" you then don`t have to keep hassling the client if she pops in every week for a certain treament! CLient will often forget to tell you if something does change; so I sometimes just quickly ask them "has anything changed with your health etc?..."

I managed to get a template which takes up the same amount of space on paper and covers EVERYTHING.
