Hi guys,

Need help on what to put on a nail extension consultation form, i am just starting out doing nail extensions and i am finding it hard to think of things to ask my clients. please help

What nail care items do you regularly use at home?

Have you a history of Diabetes, Rheumatic Fever, Heart Disease or any Allergies?

Are your hands dry, moist or normal?

What nail shape do you prefer?

Do you prefer enamel, natural-look or French?

Do you do sports activities and if so, what?

Do you have children under 5?

How did you hear about our salon?

If the client has come from another Salon and/or has problems, please ask her to sign the following indemnity.

I may have complications for which I do not hold [NAME OF SALON] responsible. I acknowledge that these Enhancements should be rebalanced every 2-3 weeks or professionally removed at a Salon. These Enhancements are not guaranteed and are my sole responsibility once I leave the Salon. Any breakages or repairs must be paid for. OOOH thankyou! but do u actually get your client to sit there and write the answers or do you go through it with them and then you write it in?

I fill in the form for them and go through it with them and then ask them to sign. I need to consult with a client personally in order for the information on the form to be of use to me. How can I fill in the 'agreement' if I haven't consulted with her?

It is important to discuss the service you have decided to perform and agree to it. What if the client wants megga long nails and you know she has 3 children under the age of 5?? Are you going to agree to that without making a note? No ..... so you need to fill in that you did not agree or recommend for her to have such long nails. When the problems start you then have a record of that conversation. This can't happen if you just give the CRC to a client to fill out with no consultation.
 
Yeah thanks i was just checking, because i never have just given the consultation to the client for them to fill in, i always go through it with them because i need to know what i'm doing and what the client wants.

thanks i was just checking, because i never have just given the consultation to the client for them to fill in, i always go through it with them because i need to know what i'm doing and what the client wants.

I'm sorry, I thought you were new to nails and were not familiar with using Client Record Cards ?? :eek:
 
No not in the whole industry im on about in the nail extensions, iv just been on a course and now I am starting to build my clients up, but was struggling to find what to ask my client because im new to the acrylics and that, so i wasnt sure so i thought id ask :)
