I have a mother that wants to have her daughters get their nails done. One is 14 and the other is 10. I think it is to young. But they have a coupon with me, so I am wanting to make a form for her (the mom) to sign.

I want the form to say something like;

Girls under the age of 18 for not full matured, their oil glands are out of control and the chemicals that are used can be harsh and harm the natural nail. Riping nails off harm the natural nail as well. Young girls do not take the time to keep up on nails. Removal should be done by a professional and be soaked off.

You would really need the advice of a lawyer on the wording and legality of such a form. Personally, I would refund the money--my policy clearly states that I will not apply enhancements on anyone under 16.

I really do not like the wording of this at all.
Firstly the information is just wrong. Plenty of girls under the age of 18 wear nails beautifully and really look after them well .. they are not all out of control walking oil bombs; NOR are the chemicals we use in nails either harsh nor do they harm the natural nails. This all sounds very scary and would put mature clients off if they read it.

Your form should be simple and informative with correct information and inviting. Maybe something like:

We are glad to offer nail services to teens along with signed parental consent.

Parents should be made aware that nail enhancements are a permanent service and need continuous up keep or professional removal.

Any damage to nails through incorrect 'at home' removal, lack of aftercare or lack of timely maintenance is entirely the responsibility of the client.
