I can't seem to find any that have a place for email addresses, any ideas? X

I did, and now my consultation cards have all the info I need for my insurance on them, along with all the marketing info for my business.

I made my own aswell. They are perfect for my needs. I print them out on card and cut them up.

Best way is to make your own and take bits from others you have seen online, remember allergies, medical history, how they heard of you, and I have a small slip on the back of mine for patch testing info, I.e their name and sig, date of patch test result and your name and sig how that helps
