Last night I drafted up a questionnaire that I was hoping to get existing clients to fill in for me, and perhaps ask some of the hairdresser clients to fill it in whilst they are sitting there.

Now, its a FIRST DRAFT!!!! So haven't really done more than put my inital thoughts down.

Would appreciate you having a quick look through and give me some guidance on how to word it and may it easier to use and attractive to look at.

this was actually done in box form so looked a lot easier to read, the yes/no/maybe was in columns so people could tick the appropriate answer to each therapy.
 
The point of a questionaire is to get the information to answers you're looking for and to encourage people to take it it has to be quick and usually have an incentive and you have covered all of those.
I think it's fine hun x
