Skin diagnosis help!!!

hey everyone, im trying to make my own skin diagnosis form for when my facial clients come in the salon.
im having real trouble on which questions to add to the form..
i have the usual questions ie, what routine do they use at home.. etc etc
anyone have any ideas? appreciated

Lifestyle - do they work in an air condioned office, or outdoors etc
diet
health

Agree totally, and I also ask them for previous salon treatments, then ask them how satisfied they have been with the products/treatments they have been/are using, i.e. were expectations met?

Yesterday I had a new facial client who admitted to using soap on her face as it's "oily" (why everyone thinks they have oily skin when it's the rarest skin type is quite odd - I reckon it's due to those who have had it as a transient skin condition & assume that's their skin type). Anyway, out came the wood's lamp, and there before my eyes was one of the most lipid dry skins I've ever seen! Not one dot of free-flowing oil & a glorious halo of light purple. Texture & the magni-lamp confirmed it.

So all this time she's been using entirely the wrong products for her skin!

underlying health concerns -

diabetes/heart problems
medications - particularly steroids, anti-coagulants, HRT
allergies
existing skin conditions - psoarisis, telangectasia etc etc

as well as all the normals - lifestyle/diet etc etc
