I cant remember who, or on what thread a member posted about using JotForm to create consultation forms; whomever it was, thank you very much.

I've found a great template to adjust for a Covid-19 statement of procedure and a liability waiver, which helped me then to think about and write my operating process.

I'm sure I shall be using it again for lots of stuff.

Under Consent Forms. I had to scroll a fair way down to get the one most appropriate but there were a fair few for different industries.
