Hi all. So I have a lady who's nails on her toes have started cracking right the way across about 2 mls from her cuticle. Underneath there looks to be new nail growing. Its happened on 3 so far. Any ideas on what's causing It?

Could be any number of reasons, but best not to provide a diagnosis and refer her to her gp instead.
 
She wore really I'll fitting shoes a few months ago and I wondered if the stress of her nails being a bit to long at the time and being it the shoes all day might have stressed them out. I'm seeing her again next week so will advise a break from gels

Unlikely to be anything to be concerned about. If its definitely another nail and not a beaus like looking like another one, then its probably damaged from an injury like shutting it in a door etc. Sometimes damage is done in the matrix or is under skin so not noticed immediately at time of injury and grows out that way instead.
If there has been no injuries and no known reason for it to happen then refer to GP.
Advise to keep short and filed to avoid snagging. It will fall off on its own.
 
This happened to my toenails a few years ago and I sent to nail clippings to be examined. It turned out I had caught a fungal nail infection!
I had to use a nail treatment on them and it cleared up.

Obviously we can't diagnose clients, but if it is a fungal nail infection it is incredibly contagious. Tell her it could be a possibility.
 
A Podiatrist can diagnose and advise on this.
We are the specialists for lower-limb and foot problems.
 
It must have been really ill-fitting shoes. Recently we moved and I walked up and down a loading ramp about 500 times over two days, jamming my toes forward in my sneakers. I was not surprised that I lost one great toenail and the other great toe acted as you’ve described your poor clients nails.

Could be one of the above suggestions or a combination. Perhaps suggest ’watchful waiting’ since you are not describing any obvious infection, pain, etc., that might warrant a trip to the Pod.

Not all infections are painful. GP services are free in the U.K. so there’s really no sense in risking leaving something to potentially get worse if it can be looked at by their doctor to rule out any serious problems.

As I said before, Podiatrists can assess, diagnose and advise on these matters. They can also prescribe antibiotics. Most offer a free 10 minute foot health check.
Our GPs are already under pressure and most are not offering face-to-face consultations at the moment unless there is a clinical need. A toe that isn't hurting doesn't fall into that category.

Oh, I didn’t realise they weren’t seeing patients face to face still.
How do they get away with that?
Here in Ireland, I can visit my GP for a face to face appointment and have done since July 2020. I know they also offer telephone appts if you request it.
 
Thank you for reminding me that the Cousins over here are waaay behind in our approach to health care.

I’ve had several discussions with clients about whether I would pay higher taxes and receive health care or keep paying private insurance premiums. I am 74 and hub 75. Together we pay over 700$ a month which doesn’t include prescription.

A favorite response is a doctor saying that we‘ll wait and watch. I now see that I’ve been programmed to feel the same.

Thanks for all the replies. I've managed to get a couple of photos. Its happened on 3 nails all together. Theres no sign of discolouration or anything at fault with the nail bed. The polish was put on a few weeks ago. And i havnt removed it as that nail is so loose at the left side but fully attached on the right. The other one came off when she was in the bath.

Had your client had Covid?
I had one toenail look like it had a fungal infection which started about a week after my positive test. It's growing out really slowly and none of the others are affected so fairy sure it's not a fungal infection.
 
Not that shes aware of. She has altered the dose of some of her regular medications a few months ago. I dont know what to do with it. Its fully attached at one side and completely off at the other and the bottom.
 
Trinity - Covid Toes are a specific Covid-related condition. The usual presentation is akin to chilblains COVID (coronavirus) toes: Definition, cause, treatment, and more

Beauty Lady - if you don't know what to do with this lady's toe, please refer her to a podiatrist as I suggested before.

I didn't get the generally accepted 'covid toe' symptoms, no swelling, pain, blisters, discolouration, etc. just a patch of discolouration on the one nail that looks like a fungal infection.

ETA - picture added, so hard to photograph your own toes 😅. Ignore the black mark under the epinychium, I subsequently stubbed my toe on the bed (covid exhaustion made me clumsy for months) and I think as it was already damaged it bruised under the nail.

I mean for getting the gel off. Not sure if a podiatrist does that. Theres literally nothing unusual to look at. Just completely loose at the bottom and only attached in the right hand corner on that photo.
