Photo release forms

Hi does anyone get there clients to sign a form if they take pictures of them before and after treatments. Say if I'm doing someone's make-up that I want to put in a portfolio, should I get them to sign something? If I do what is it I have to get them to sign?

I have it at the bottom of my consultation form, they tick the box if they agree for the photos to be used for my marketing x
