Rent a chair - Contracts

So fellow geeks Ive gone ahead bit the bullet and I am opening my own salon ( said a excited but business headed 21 year old)!
I am running it as a rent a chair basis and would like to know if anyone has any templates of their rent a chair contract or agreements.
Just to give me a rough idea as to where to start.
