Refer her to a specialist, either a dermatologist or trichologist. There are many underlying causes of alopecia and needs a specialist to diagnose/treat.
I would be very careful about applying extensions to her hair until she has been told this is ok.
Has she not been to her doctor yet?

looks like alopecia i'd get them extensions out and not fit any more

She hasn't been to the dr. She only discovered it this morning and said she's never had issues before. I have told her to get the extensions taken out pronto and that I can't fit any more. She said she's had the micro rings in for about 10 weeks and did have a maintenance appointment. Do you think it was caused by the micro rings in some way. I'm obviously concerned about this happening to my own clients. Thanks x

I Doubt its caused by extensions, looks like alopecia areata to me with the big smooth patch.
Definitely send her to docs tomorrow poor girl.
Surely shed noticed all the hair/extensions come out? She should ask the other hairdresser if it was there last time she did her hair x
