Client record cards

I need to make some client record cards, but i dont know if i should make them seperate for beauty and massage, also does anyone have a template that i could maybe use as everything I want to ask is going to take up 5 pages!!

Its for a Hotel, We have most clients details, but I would rather a beauty card to cover myself, however the massage therapist wants something different, although in my salon in england I just had the one for all treatments and one for sunbeds.

I wanted;
Personal details
medication
surgerys
medical history (H-attack, diabetes, varacus veins etc etc)
allergies

then on the back i will have my bit for pre consultation, skin anaysis, products sold etc

but the massage therapist wants every illness medication injury and everything which takes up alot of pages and a client doesnt want to read that much info..... maybe i am wrong?!
