New client pack/form

I am just setting up mobile and want to know what people do when it's new clients and what do they put in the new client pack? Obviously all their personal info, any allergic reactions etc.. I have tried searching on here but not found anything. Thanks

I put in a new client pack a few leaflets from local businesses, a welcome letter, freebie gift, loyalty card etx

Im not sure what you mean exactly by new client pack.
My insurer has downloadable templates for consultation forms and treatment records so thats what im using.
as for when I get a new client, I have a 10% off your first treatment offer and a loyalty card, also a full price list because I couldn't fit it all on my flyer lol
