Spray tan consent forms

Hi, I have a group booking for a dance show... 14+ year olds.
I am looking for a consent form for parents to sign... Does any one have any templates as such?
I believe a parent / guardian has to be present.
Do they have to wear tops/bras or can I get parents to sign that they wish for me to do it without top/bra as they need a full tan for the shows they are doing!!

Confusing!!!! Haha

I'd speak to your insurance company. When I trained in tanning I was told you never allow them to remove tops under the age of 16. Having spoken to my insurance company, they seem to be fine as long as an adult is present at all times. This protects you and the child.
