Client responses?

I was wondering if someone could help me? I cannot find the answers i need ive been stuck on this question for hours, Why is it important to record client responses to questioning?
if someone could point me in the correct thinking wave that would be great!
I dont want the answer as such but some guidance of what the answer could be.

You record the clients responses to prove that you asked.
E.g. in a consultation, you record that you asked them all the relevant health questions to check for contraindications to treatment.

And

If the info is written down then other therapists that treat that client are aware and don't have to ask the consultation questions each time the client visits.

Also there could be weeks/months between appointments and it's impossible to remember any contraindications for each client, with a record card you can simply skim over the answers before the client arrives and be ready to go.
