I have just contacted my insurance company to check if I will be covered for treatments on children (ie manicure and pedicure parties for little girls) and they said yes but I will have to get the parent of any child to sign a consent form before treatment takes place so if anything was to happen as a result of my treatment and a claim was to be made against me I would need to send this consent form to the insurance company.
My question is what sort of wording would I need to put on the consent form?
