consultation cards... please help!

hi all, i have spent the last 3 hours researching client cards for spray tanning and make up... i would ideally like to download them, if anyone knows where i can. i dont want standard printed ones from the warehouses, i would like a proper consultation card, with contra indications to treatment and disclaimer if poss. please help before i tare my hair out..

have you tried itec web site they have make-up clients forms. and i'm sure you could change them to suit the spray tanning.ITEC - Beauty Qualifications if you go to this page you will see beauty specialist click on this for make-up forms and you can print them off. also on the page is tanning treatments again click on this and you will see forms too. hope this helps.

i buy solglow from my local warehouse ewd. they dont sell them unfortunately!
