Consultation forms/patch test

Hi! I’ve just ordered some consultation forms from lash base.
Do I need to complete one of these for each service eyelash extensions, tinting and lifting.

Also for a patch test form, can I have a universal form and just list/sign that the client has had a patch test?

What other forms will I need?

I would recommend looking and speaking to your insurance company. Each are different and will require different things from you.
Personally I have to record my patch tests all on the same consultation form for each treatment. I am also ok to do multiple treatments on an individual consultation form as long as they’re in the same “area”. For example, pedicure and manicure can be down in the same, all eye treatments can be done on the same, facial, massage and reflexology is all separate.
But like I said, u need to check with yr insurance company. Hope this helps x
