Consultation forms

Hi all,

where does everyone get consultation forms from , ?? Can you get free ones from Somewhere or do you need to purchase these. ? I require forms for all different treatments

facial
pedi etc
eylash etc

Hi KK OnHols,

I used to buy 3 different types, General beauty, Electrolysis and Holistic therapies. I have made my own based on my more detailed needs and I simply circle which treatment the client receives from me.
It is full A4 sheet and has lots of space for all the necessary medications list, surgeries, ailments etc. It is far better than any shop bought card. Masses of room on the reverse side for date, treatment , cost or anything to report etc. Once folded it still fits into my file with the name easily visible for quick retrieval.

I do still have cards which I get from Alan Howard but my home made consultation form is really good.
