Hey!
Iv created my own record cards and at the end after asking them all about their health and allergies etc etc, i've put this paragraph
'I hereby declare that all of the information I have given is up to date and accurate. If I should react at all in any way to the treatment I am about to receive due to lack of information given, I will not hold So Be responsible.
Signed: Date:
Do you think this is ok and covers me if they dont tell me anything and the treatment affects them?
Is there a basic statement anywhere that i can use instead?
