Example letters

Hi, I am currently training for Swedish massage, I have been asked to provide a couple of example letters:
1 A letter asking permission to a parent to massage their child.
2 A letter asking permission for a childs case history.

I have no idea where to begin, as I am from an office job background and know noting about this at all!

I am in exactly the same position hun. Im doing Swedish massage and also work as a secretary. I really dont mean to be funny but you cant ask these girls on here to do your work for you. I have to get my thinking cap on, use the t'internet and use common sense to work it out.

Like I say, Im not being funny but sometimes you just have to do the work yourself.
