Marketing questionaire-any ideas please?

hi, I would like to come up with a questionaire to basically find out if there is a 'interest' within the local area for a beauty room (where I am considering setting up within the Pharmacy).

I was thinking of targeting the Pharmacy and the Hairdressing salon next door - the only two shops in the area.

I would be very grateful if anyone has any ideas on what Q's to ask, how to word them, how many questionaires to put out and for how long...and anything else you can think of.
