HI Again,

After much delibiration (if that's the word and the spelling) I've got a 13 year old booked in for a set of acrylics i've spoken to both parent and child and explained i would need her to sign a parental consent form.

Given I have never done this before does anyone have a template or a few points for make up the parental consent form?

I don't have a consent form for treating young clients but thinking it is a really good idea to have a generic one on hand for any services that we have. There are things that can go wrong and even though we explain things etc, sometimes the clients are young! I think when it comes to the young there are many issues, including the opinion of the establishment (i.e., boss, manager...for and agains't treating them), and the opinion and comfort zone of the therapist that performs the treatment (for and agains't), not to forget the parents informed opinion. It seems to be a very fine line that involves personal morals and beliefs etc.
I guess that for the most part I would have something in consent form that states treatment, that it has been explained pro's and con's, risks, maintenance...and I am sure there is plenty more that can be added!! :rolleyes:
