Payot Skincare order form

Hi all,

I've just taken over a salon that used to stock and use Payot. I've switched to another brand and been told I can return the Payot to them and they'll credit my account with the value of stock returned.
The only problem is that I need to send a Payot order form with the stock to prove the stock value, does anyone have one that they'd be prepared to email to me?
