Stock monitoring documents

hi all, just wondering if anyone has an effective stock monitoring system document that i could use as a template? i have tried to do one myself but doesnt look right... if you know what i mean...

Make your own. Take all your invoices that you began with and make a list of every product that you have purchased then its up to you as to how you want to measure them.
I have the items, how many I need on premisis/re-order level, stock take date, amount to order and order date.

eg. cream puff - 3 - 1 - 16/3/09 - 2 - 18/3/09
Cream puff is item name, I keep 3 bottles in salon, I have 1 bottle on monday and will buy 2 more on wednesday.

this is my own one and it works for me other geeks may have much better systems.
