Struggling with getting Swarovski's to stay put!

I use Gelish and I'm finding it a bit hit and miss with my Swarovski's staying on. Iv put them in the top coat, cured then top coat again. Iv also applied Gelish then used UV gel to stick them on, cured and then a top coat. Iv even gone round them again with uv gel. Sometimes they stay on sometimes they don't. I did my nails last night and one has come off already however Iv had them stay on for over 3 weeks! What am I doing wrong? Any help or tips would be great!! X
