Can you remind me how to create this heart?

Do you carve the heart out with a dotting tool??

I use shellac (all shellac products) been trying to find a tutorial on it in YouTube but nothing has come up x

I love this "negative space" nail art. Yes I would carve with a dotting tool, then with a barely moistened small brush with IPA, sweep out rest of it.

If it's a negative space heart then yes you'd need to carve it out. If not then it's entirely up to you, you could carve out or free hand it xx
