Childrens pamper party parental consent

Ive got a few bookings for pamper parties & its my intention to send out consent forms for the parents to sign before the party. However, Im not really sure how to word the consent form. Would anyone be willing to let me see their forms or give me an idea of how your form is written please?

I have an A6 piece of paper saying

I give consent to .........( company name) to apply on (date) ..........
Nail varnish. Yes / No
Glitter. Yes / No
Gems. Yes / No
Nail wraps. Yes / No

Child's name .........
Date of birth .........
Allergies ........
Parent name/signature ........

(if I do not have your consent I will not give treatments to your child)


I give these to the parent of the party child to hand out with the invites
