I'm a semi new esthetician and I'm opening my own skin studio (very small scale, just one room) and I'm looking for a client intake form for a facial. I'm also curious if its standard for clients to fill one out prior to a back treatment or body scrub as well as prior to a facial?
I would love it anyone could share a form that is I could revise as needed.
