Client treatment form

Hello All! I'm hoping to start working from home in the new year and am partway through my massage/facial training. The college i go to doesnt supply client treatment forms. Does anyone have an example of one that I could look at so that I can create my own? I would be really grateful for some info! It would be for facials/makeup/massage. Thanks guys. XX:lol:
